Health Requirements

Illinois Department of Public Health Immunization Requirements

Each school year, all students must be up-to-date with physical examination and immunization requirements. All completed forms can be submitted using the following link: click here or can be dropped off at your child's school health office. Students who have not submitted their required health documents by the first attendance day of school may be excluded from school until notification has been received by District 3 that the required health documents have been provided.

Physical Examination Information:

  • All students entering early childhood, kindergarten, and 6th grade are required to have a current physical examination completed via the approved Illinois Department of Public Health Form. The parent portion on the upper backside of the form must be completed and signed. 

  • A sports physical does not meet the physical requirement.

  • A current physical is one that was completed within one year of August 1st of the said school year, for example: for the 2023-24 school year, the physical must be dated within one year of August 1, 2023.

Immunization Requirement Information:

  • All students must be up-to-date with the State of Illinois immunization requirements.

  • All students entering sixth, seventh, eighth, ninth, tenth, and eleventh grade must show proof of receiving one dose of Meningococcal vaccine (meningitis).

  • All students entering grades six through eight grade must show proof of receiving one dose of the Tdap vaccine. 

Athletic Physical (Middle School Only)

Any student trying out for an athletic activity (middle school only)  such as an  interscholastic team, cheerleading, or dance must have a current physical examination form on record noting approval for interscholastic sports. The examination must be on file with the school PRIOR to tryouts and current through participation in the sport.   

Physical Examination and Immunization Requirements by Grade (Preschool - 8th Grade)

Preschool

Complete Preschool Physical Examination.

No additional immunization requirement if up-to-date.

Kindergarten

Complete Kindergarten Physical Examination and all required immunizations including:

- Two does of MMR Vaccine

- Four doses of Polio Vaccine

- Four doses of DPT/DPTaP Vaccine

- Two doses of Varicella Vaccine (chicken pox)

Complete Kindergarten Dental Examination - Due the first day of school. The student will be excluded from school if it is not received by May 15 of the school year.

Complete Kindergarten Vision Examination - Due the first day of school. The student will be excluded from school if it is not received by October 15 of the school year.

1st Grade

No additional physical/immunization requirements if up-to-date.

2nd Grade

No additional physical/immunization requirements if up-to-date.

Complete 2nd grade Dental Examination - Due the first day of school. The student will be excluded from school if it is not received by May 15 of the school year.

3rd Grade

No additional physical/immunization requirements if up-to-date.

4th Grade

No additional physical/immunization requirements if up-to-date.

5th Grade

No additional physical/immunization requirements if up-to-date.

6th Grade

Complete 6th grade Physical Examination and all required immunizations including:

- One dose of Tdap Vaccine

- One dose of Meningococcal Vaccine (meningitis) after the 11th birthday
(If your child turns eleven after August 1st, you must submit proof of an upcoming scheduled appointment)

- Two doses of Varicella Vaccine (chicken pox)

- Three doses of Hepatitis B at the required intervals

Complete 6th grade Dental Examination - Due the first day of school. The student will be excluded from school if it is not received by May 15 of the school year.

7th Grade

No additional physical/immunization requirements if up-to-date.

8th Grade

No additional physical/immunization requirements if up-to-date.

Health Forms

Now is not too early to begin scheduling important physical and immunization appointments! All students must be up-to-date with physical examination and immunization requirements and all completed forms must be turned into your child's school health office by the first day of school. Begining October 15 of each school year, students that are not up-to date will be excluded from school.

Dental Examination Requirement

All students entering kindergarten, second, and sixth grades are required to have a dental examination. Dental examinations are due by the first day of school, and the student will be excluded from school if it is not received by May 15th of the year the student is enrolled in kindergarten, second, or sixth grade. A current examination is one that was completed within 18 months of May 15th.

Eye Examination Requirement

All students enrolling in a kindergarten program or enrolling for the first time in a public school shall submit proof of an eye examination completed within the previous year before October 15th of the school year.
